Acne scar revision is a medical procedure. Make no mistake that even in the best of hands with extra precautions, unforeseen adverse events can occur- my most common problem is post inflammatory pigmentation, followed by infection. Other potential side effects include arterial occlusion, nerve damage and idiosyncratic scarring- all extremely rare side effects. If adequate follow up is not within my control, I often decline to treat patients. My ethos is safety before results. These are the reasons why I, or some dermatologist or plastic surgeons may elect NOT to treat you- 1. First do no harm. That is rule of medicine and our oath. If I perceive the risk benefit ratio is too high I will not treat you. This means even if you sign a waiver I will not do it. An example is to treat acne or chickenpox scars with say Sculptra, fat transfer, Radiesse or even Artifill or Bellafill in the forehead. If I take all the precautions, I still may hit an artery, and this may lead to permanent blindness. I will use an HA filler but not any others. 2. If our goals are not aligned – namely if your goal is have flawless glowing skin, however my goal, due the limitations of both my skill sets and technology can ‘only’ improve 90% of your scars. 3. If I perceive that this treatment is beyond my skill sets & YOU choose to be a client and NOT a patient. My treatments in the context of acne scarring is medical & not cosmetic, hence I only treat objectively measured acne scars. For patients who have subjectively severe scarring I will still work on your scars, providing you do it my way- this is in your best interest. 4. If I perceive that there maybe an element of dysmorphorbia AND you are not willing to work with me to help you, this overlaps with the above. Acne scarring is more than just physical- its mental & emotional scarring. The majority of patients with severe scars can see improvement as they are positive in their thinking. If you are a negative thinker, & despite objective improvement that you fail to see improvement, I may elect to work in with a psychologist; once again in your best interest. 5. If you are rude to my staff, including both reception or my nursing staff, this may compromise patient care. This reflect upon the outcomes and my perceived abilities. I work under a corporate entity, and if my staff are rude to you in any way, the ‘big guns’ namely the company, not myself will deal with that. Being rude to people is disrespectful, and may reflect a personality trait that I do not want to work with. I enjoy my work immensely. Only a very small minority of patients make my day painful. There are many other dermatologist who will see you for money, I am one of those. 6. If you take your own post operative care into your own hands or ignore my post op follow ups.Checks are done for a reason, - safety. I do not charge for wound care, nor low level laser healing sessions, unlike most dermatologists. My aim is to heal your skin up as quickly as possible & discharge you back home. I am meticulous with wound care. If you turn up for checks, or keep in touch with my staff with emails & photos for my many International patients, I can minimise side effects. In 2019 I will introduce new changes to my practice to try to eliminate the 3-5% of patients that take up so much of my time. This gives me more time to conduct research, complete papers and spend more time on the many patients that I enjoy treating. I am passionate about my job, but also understand I have a duty of care, if you think I do not live up to your expectations, or if you are a total ars*hole I will give you a report, but will refer you on to other dermatologist who will gladly see you- Google ‘Acne Scars’ in your City & you will find these dermatologist. For those trolls and angry patients on every website or Instagram who dislike every post I have from skin care tips to my supposedly fake before and after photos, the 100k challenge is still there.
